We worked with Age UK to produce this short film which tells the story of how joined-up care at Weymouth Hub in Dorset improved the lives of Dennis, who was living with multiple complex conditions, and his wife Jennifer, who cared for him. It shows how the NHS and local councils are working together to improve health and care through sustainability and transformation partnerships (STPs).
Dennis passed away in Autumn 2017, and is survived by Jennifer and his children. This updated version reflects this. With Jennifer’s support, we are continuing to tell this story to help people understand the benefits of improved services. The NHS is grateful for her continuing support.
